In-Home Medical Alert Review
Our in-home test unit arrived with a base station featuring a 32-hour backup battery and a standard necklace button. We also added Bay Alarm Medical’s fall detection pendant to see how it performed in terms of comfort, ease of use, and sensitivity to false alarms. Since more people are moving away from landlines, we opted to try the new 4G LTE cellular version. That said, if you still have a landline, we recommend using it, because you’ll save some money and the reliability is excellent.
If we had one criticism, it’s that the equipment looks a little dated. Bay Alarm Medical explained that the design is intentional: the plastics are shaped for optimal acoustics, built for durability, and easy to wipe clean. All valid points, though we still found ourselves wishing for a more Apple-like aesthetic.

That being said, the materials are indeed rugged and easy to maintain. Both the pendant and base station buttons offered solid feedback when pressed, were waterproof, and light enough to wear all day without issue.
From our Bay Area headquarters, the cellular system worked flawlessly, delivering the same performance as the landline setup. We didn’t encounter any connectivity problems on AT&T’s network, though rural buyers should always check the coverage map before ordering.
As for the fall detection pendant, we couldn’t trigger any false alarms during testing. A Bay Alarm Medical rep, Jacky, reminded us that no fall detection technology is perfect. For $10 a month, we view it as an added layer of protection, a smart backup, but not quite as solid a helper as pressing the button if something actually happens.
Setup
Installation was surprisingly fast – about four minutes from start to finish:
1. Unplug your home phone from the wall jack
2. Connect the base station to the wall jack
3. Plug your home phone into the base station’s “phone” port
4. Plug in the power cord
5. Turn on the base station
6. Run a test
With the cellular option, setup is even easier since no phone line is required. Once powered on, the buttons were already pre-programmed. A quick press placed us in contact with a Bay Alarm Medical operator, who addressed us by name and confirmed the system was working. We tested coverage throughout a 2,000 sq. ft. home and never had an issue—the operators were consistently professional, friendly, and clear.
For best results, we suggest placing the base station near the center of the home, or near high-risk areas like kitchens and bathrooms. The system’s speaker is loud enough to cut through background noise, even with a closed bathroom door and running shower.

Medical alert systems aren’t something most people want to use—and convincing a loved one to wear a pendant can be tough. For stubborn family members, wall-mounted buttons can be a good alternative. SOS All-In-One 2 Review
Bay Alarm Medical’s SOS All-In-One device has long been a favorite for those wanting protection both at home and on the go. In 2024, the company rolled out the updated SOS All-In-One 2, and the improvements are noticeable.
The new version includes an interchangeable in-home button that pairs with the mobile device while it charges in its cradle. Automatic fall detection is available as an upgrade. The device now comes in black or white; we liked the satin white with red accents, though the matte black is better for hiding smudges.
Compared to the previous model, the AIO2 is sleeker, more water-resistant (IP67 rated), offers longer battery life, and has better GPS tracking and fall detection.

Another standout feature is Caregiver Tracking, which is included at no extra cost on all of the SOS mobile devices. Through the companion app (available on iOS and Android), caregivers can check the device’s location – useful for peace of mind or even for finding a misplaced unit.
Battery life was solid. We consistently got two full days of use with a little left over, though we still recommend charging it nightly. The included in-home button makes it easy to cover sleeping and showering periods when the device is charging.
Overall, this is our favorite Bay Alarm Medical offering. The only tradeoff is that its speaker isn’t quite as powerful as a dedicated in-home system due to its compact size. Still, as an all-around solution, the SOS All-In-One 2 is hard to beat.

SOS Mobile Review

SOS SmartWatch Review
Bay Alarm Medical also broke new ground with the SOS Smartwatch, the first medical alert smartwatch to offer fall detection. Designed as a stylish alternative to traditional systems, the watch combines a sleek, lightweight design with practical features: a bright, easy-to-read display, long battery life, water resistance, two-way communication, and most importantly, a physical SOS button. Interestingly, Bay Alarm Medical intentionally designed the SOS Smartwatch to be the opposite of an Apple Watch. Pricing and Value
Of all the medical alert systems we tested, Bay Alarm Medical came out the most competitive with a starting price of $27.95. And unlike their competitors where you have to commit to a longer term if you want a lower average monthly price, Bay Alarm Medical starts at the $27.95 base price no matter if you decide to pay month to month, quarterly (3 months at a time) or annually (12 months at a time). And since we purchased an additional wrist button, both my parents had access to monitoring and there wasn’t an additional monthly monitoring fee that we had to pay.
Note that all three SOS mobile devices require need to be purchased outright, unlike the in-home consoles which are leased and need to be returned about service cancellation.
